Prepare before applying

Documents and information you may need

Current city or county business license, as applicable
Current Georgia mobile food service unit permit
Authorization to operate in Paulding County when your county of origin is outside Paulding County
Current food safety manager certification, such as ServSafe
Current certificate of general liability insurance
Menu with pricing, truck photographs and food photographs
Truck dimensions, power and water needs, fuel information, and waste-disposal plan

Important: Requirements can vary with the operator, menu and county of origin. The Dallas Markets' application is not a health-department permit and does not replace approvals required by Georgia, Paulding County, the City of Dallas or other authorities.

Required before vending

Download and complete the DPH authorization form.

In addition to completing The Dallas Markets application, every applicant must download the official Mobile Food Truck, Trailer, or Pushcart Authorization form.

The completed form must be returned to The Dallas Markets after it has been completed by both the food-truck operator and the appropriate Department of Public Health authority. No applicant may vend at The Dallas Markets until we have received the completed form and all other required documentation.
